What Is a Wedge Resection?

A wedge resection (also called a partial nail avulsion) is a minor surgical procedure used to treat ingrown toenails. It involves removing the offending edge of the toenail and treating the underlying nail matrix to prevent regrowth in that area.

Assessment and Consent

Dr Ho will examine your toe, discuss the procedure, and answer any questions. Your medical history — including any medications, allergies, diabetes, or circulation issues — will be reviewed. Written consent is obtained before proceeding.

Local Anaesthetic

A digital nerve block is administered at the base of the affected toe using local anaesthetic. This numbs the entire toe. You may feel a brief sting during the injection, but the toe becomes completely numb within a few minutes.

Wedge Removal

Using sterile instruments, the offending nail edge (the "wedge") is carefully separated from the nail bed and removed. This is the portion of nail that has been growing into the skin and causing pain.

Phenol Cauterisation

Phenol (a chemical agent) is applied to the exposed nail matrix — the area where the nail grows from. This destroys the nail-producing cells in that specific area, preventing the removed section of nail from regrowing. This is what helps reduce the likelihood of recurrence.

Dressing and Aftercare

The toe is cleaned, an antiseptic is applied, and a sterile dressing is placed. You will receive detailed aftercare instructions before leaving the clinic. The entire procedure typically takes 10–15 minutes.

Recovery and Aftercare

Recovery from a wedge resection is straightforward. Most patients return to normal daily activities within 1–2 days.

  • Day of the procedure: Keep the dressing clean and dry. Rest with the foot elevated where possible. Take paracetamol or ibuprofen if you experience any discomfort.
  • First 1–3 days: Change the dressing daily as instructed. Wear open-toed footwear or loose-fitting shoes. Avoid strenuous exercise, swimming, and prolonged standing.
  • Week 1–2: The toe will gradually heal. Continue regular dressing changes. Keep the area clean with gentle saline or antiseptic soaks as directed.
  • Week 2–4: Most patients are fully healed by this stage. The nail will appear slightly narrower than before, but this is cosmetically subtle and rarely noticeable.
